Patient Consent Form For Transport In The Event
Of An Emergency

In the event of a projected major storm or storm surge, our primary responsibility will be for our patient’s safety. If an emergency contact is not available to pick up my pet, for some unforeseen reason, I authorize the owners and agents of WVP of Louisiana, LLC (Pontchartrain Animal Hospital) to transport my animal to Hillside Veterinary Associates, LLC, located at 78290 Highway 437, Covington, LA, for care during my absence. This will only be done out of absolute necessity (i.e. life-threatening flooding or other unsafe storm conditions).

All animals will be returned to Pontchartrain Animal Hospital as soon as conditions are safe to do so. Accommodations will be made to pick up your pet earlier than our anticipated return date, if needed.

We will notify all pet owners of transportation needs using the provided contact numbers on your boarding/hurricane consent forms. Please ensure that all information provided is correct.
