Hurricane Season Release Form

It is in the opinion of Pontchartrain Animal Hospital that neither people nor animals should be left in the path of a major hurricane. Should your pet be housed in our building during a period of evacuation, you should be fully aware that routine boarding care cannot be accomplished. Pontchartrain Animal Hospital will not endanger the lives of its employees! We will attempt to be the last to evacuate and the first to return, but the unpredictable nature of a hurricane dictates that we are not in control. Your pet may be left unattended for an extended period of time. If our building does not survive the storm, your pet may also not survive! By leaving your pet in our facility during a hurricane, you are assuming responsibility for the outcome. In the event of a recommended evacuation of Slidell under hurricane warning, I authorize and have made arrangements with the following persons to pick up my pets from Pontchartrain Animal Hospital when notified to do so:

Please have them contact Pontchartrain Animal Hospital as soon as Slidell is placed under a hurricane watch to receive further instructions. If your animal is involved in an evacuation from our facility, an additional fee (of undetermined amount) will be assessed to your boarding fee. I hereby release Pontchartrain Animal Hospital from any and all liability in the event my pet is not picked up when notified during a hurricane evacuation. I have read this document in its entirety and agree to all terms and conditions stated herein.
